WHAT IS A COVIDCARD?

COVIDCard is a convenient version of your official CDC Vaccination Record Card. It is durable, waterproof, and fits in your wallet for easy carrying and storage. COVIDCard provides proof of your COVID-19 Vaccinations.

WHERE DO COVIDCARDS COME FROM?

COVIDCards are authorized, printed, and shipped from a government approved CDC Vaccination Provider in Philadelphia, Pennsylvania. For more information on our parent company, click HERE.

DOES COVIDCARD REPLACE MY PAPER CDC VACCINATION CARD?

COVIDCard is a supplement to your official CDC Vaccination Card. For day-to-day life, such as showing proof of vaccination at a restaurant, your COVIDCard is sufficient. 

WHAT SHOULD I DO WITH MY ORIGINAL, PAPER CDC VACCINATION CARD?

We recommend that you store your original, paper COVID-19 Vaccination Record Card in a safe, dry place, such as a safe in your home, as you would your passport or birth certificate.

ORDERING

I DO NOT HAVE MY ORIGINAL CDC VACCINATION RECORD CARD. CAN I STILL ORDER?

You will need to take or upload a picture of your original card to order. 

To obtain a replacement CDC Vaccination Record:

  1. Contact your original vaccine provider

  2. Contact your city or state's health department

WHAT HAPPENS IF I NEED TO ORDER ANOTHER OR A NEW CARD?

If you have already ordered and need another card due to new eligibility guidelines published by the FDA and/or CDC, please email covid@centennialpharmacy.com with your name, phone number, and original order number to receive a 40% off code.

If you have already ordered and need to order another card due to losing your card, we cannot offer you a discount code at this time... We're making it AS EASY AS POSSIBLE to carry vaccine proof around to get back to normal! It FITS in your wallet!! Keep it there!!!

CAN I ORDER FOR MULTIPLE PEOPLE AT ONCE?

Due to HIPAA protections and security, each order must be unique to one individual.

DO YOU OFFER DISCOUNTS ON BULK ORDERS OR COMPANY PROGRAMS?

Discount codes may be provided for organizations looking to offer programs for 100 employees or more to receive a Wallet COVIDCard. Please email covid@centennialpharmacy.com with your name, phone number, and description of what your needs are.

WHAT IS YOUR REFUND POLICY?

If, for any reason, you are unsatisfied with your Wallet COVIDCard, please email covid@centennialpharmacy.com with your name, phone number, order number, and reason you are dissatisfied. If we are unable to resolve the issue to your satisfaction, you will receive a refund of your card, minus shipping and administration fees.

SHIPPING AND DELIVERY

WHEN WILL I RECEIVED MY WALLETCOVIDCARD?

Please allow 7-10 business days for your card to be produced and shipped.

CAN I TRACK MY ORDER?

At this time, we cannot offer tracking. If you have not received your card in 10 business days, please email covid@centennialpharmacy.com with you name, phone number, and order number.

OH NO! I PUT THE WRONG SHIPPING ADDRESS IN! CAN I MAKE CHANGES TO MY ORDER?

You may make changes to your order with 48 hours from your purchase. Please email covid@centennialpharmacy.com with your name, phone number, order number, and change needed.

HOW DO YOU SHIP?

Your COVIDCard will be shipped by USPS First-Class Mail or FEDEX in a security envelope. Every order will be shipped in its own envelope.

WHERE DO YOU SHIP?

Orders can ship to all 50 States in the United States of America. We cannot provide cards to other countries.

HOW IS MY PERSONAL INFORMATION SECURED?

WalletCOVIDCard and COVIDCard use industry-best practices to ensure the highest level of security and safe-guarding of your information. All data received is encrypted in accordance with HIPAA secure protocols. 


Being an actual Vaccination Provider, we are a HIPAA-covered entity, with our entire team HIPAA trained and certified to ensure the protection of your personal and medical data.

HELP! THE CAMERA TOOL IS NOT WORKING!

First, make sure you 'ACCEPTED' camera permissions. 

If you were not prompted to allow camera permissions, or have allowed them and the camera is still not working, please attempt to use a different internet browser or another device with a camera.

MY PICTURE IS NOT IN FOCUS. WHAT SHOULD I DO?

If your picture is not clear, please click the 'retake picture' button at the bottom of the screen and take your picture again. Helpful hints: Clean your camera lens, use a dark background, take the picture by a window for light, and hold your phone with both hands over your card.

I DO NOT HAVE A DEVICE WITH A CAMERA. WHAT SHOULD I DO?

Proof of valid vaccine records are needed to order a COVIDCard. If you do not have a device with a camera, please fill out the prompted vaccine information section, and a HIPAA-Certified member of the COVIDCard team with contact you and/or your vaccine provide to obtain proof of vaccination.
